CHW Program

What is a CHW? A community health worker (CHW) is a member of the community in which they serve.

They provide basic health and prevention care. They are able to help advocate for the clients needs and translate medical jargon. They also help to find much needed resources for their clients. Our CHW’s currently serve people with COPD, Pneumonia or Health Failure that have been hospitalized. We provide services after the patient is discharged to help keep them from being readmitted so that they can increase their quality of life.

Grants

2017-2020 HRSA Network Development Grant

This grant awards $300,000/year to Purchase Area Health Connections over the next 3 years. This grant will help 1) to continue to strengthen the Network and its members and 2) to improve population health and reduce hospital readmissions by developing a joint system of transitional care. While transitional care program will contribute to the Network’s strengthen and sustainability, building the Network’s capacity will contribute to the strength and sustainability of the transitional care. Our target population includes individuals discharged from two partner hospitals with a diagnosis of COPD and/or Pneumonia living in the service area that includes: Ballard, Calloway, Carlisle, Fulton, Graves, Hickman, McCracken, and/or Marshall counties.

2019-2022 HRSA Small Health Care Provider Quality Improvement Program

This grant awards $200,000/year for 3 years. Our goals are: 1) to improve overall patient health outcomes in our target populations; and 2) develop the capacity of the PAHC Network to use Evidenced-based Quality Improvement Practices that results in a sustainable model for CHWs in the community. When successful, we anticipate that avoidable hospitalizations ill be reduced, health outcomes will improve, patient satisfaction with care and health literacy will improve. Our target population includes individuals discharged from two partner hospitals with a heart-failure diagnosis living in the service area that includes: Ballard, Calloway, Carlisle, Fulton, Graves, Hickman, McCracken, and/or Marshall counties.
